The Creative LifeWork Design Life Coaching Agreement will be emailed to you by Life Coach Nancy J. MIller with an invoice or Paypal link for payment of services or you can download the agreement below, sign and return by email.
This Life Coaching Agreement is between Nancy J. Miller, Career Counselor/Certified Life Coach, and
______________________________ (Client) for Life Coaching Services. Life Coaching is a working partnership, not a legal business partnership, between your Life Coach and you the Client, in an informative creative process that inspires you to maximize personal and professional potential. Any information in this agreement can be discussed and questions asked by you, the Client, before agreeing. A clear mutual understanding in speaking and writing is important for a successful coaching partnership.
Appointments
Appointments will be scheduled as needed and paid in single payments as arranged by your Life Coach and you, the Client. No additional charges will be made without consent and agreement with coach and client.
The time, place, or format for coaching sessions will be determined by Coach and Client. Your Life Coach, Nancy J. Miller, will initiate all scheduled calls from phone, or provide a Zoom link, whichever method is agreed upon. You, the Client, will provide your phone number where you will be available for agreed upon communication, and your email address for appointment Zoom links and follow up correspondence.
You the Client, agrees that it is your responsibility to notify your Life Coach at least 24 hours in advance of any scheduled call/meeting if you the Client, cannot make the session. You, the Client, will be billed for a missed session if you do not meet at the scheduled time or reschedule. Your Life Coach will wait for 15 minutes, before closing the session if you the client, has not called/come and any missed time will be forfeited.
Confidentiality
You the Client, understand that everything said in coaching sessions and emails are confidential as much as possible within the limits of using the telephone, the internet, or public places. Documents, resumes, letters, or any material sent to me, your Life Coach, will not be shared with any person or place without written consent by email or print from you, the Client.
Emotional Freedom Technique
Emotional Freedom Technique (EFT) with the Client tapping on their acupressure points may be used in coaching sessions with the Client’s consent if the Coach feels it would be helpful for working with stress, tension, a problem, or accumulated small trauma’s that do not require a diagnosis. This is a fairly new approach to working with the human energy system and that is understood to affect the body/mind interconnection. Scientific studies are confirming that these approaches can assist in enhancing relaxation, increasing a sense of well-being, and reducing pain sensation. To the Coach’s knowledge, there are currently no known side-effects to energy-oriented treatments when properly administered by trained practitioner.
You, the client, further understands that because these methods are relatively new, the extent and breadth of their effectiveness, including risks and benefits, are not yet fully known.
You, the client, have the right to request or cease using these approaches at any time. You, the Client, agree to take full responsibility for your self-care in the physical, emotional, mental, and spiritual dimensions of your life.
Legal Responsibilities/Disclaimers
As the Client, you are responsible for creating and implementing your own physical, mental and emotional well-being, decisions, choices, actions and results. Your Life Coach, Nancy J. Miller, is not, and will not be liable for any actions or inaction, or for any direct or indirect result of any services she provides. Coaching is not a substitute for professional advice by legal, mental, medical or other qualified professionals and you, the Client, will seek independent professional guidance for such matters.
Except as expressly provided in this agreement, the Coach makes no guarantees or warranties, express or implied. Coach’s entire liability under this agreement, and the Client’s exclusive remedy, will be limited to the amount paid by the Client to the Coach under this agreement for all services rendered up until the termination date.
